Care Manager Compensation
- The pay for this position is between $36-40 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Care Manager Highlights
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- The required availability for this position is Monday to Friday 8am to 5pm PST (Must reside in PST timezone)
Care Manager Responsibilities
Outreaching member to enroll them in CM program
Building care plans, medication review, assessments care plans
Care Manager Requirements
- Registered Nurse with Active State License (CA) Must be CA regardless or state of residence
- Associates, Bachelor's, or higher degree or accredited nursing diploma
- 2+ years of clinical nursing experience with at least 1 year in Managed Care
- Strong Computer skills, including e-mail, word processing, spreadsheet and web-based programs
Care Manager Preferred Qualifications
- Home Health Experience
- Case Management experience in a remote environment
