Job Title: RN Care Manager ED

Location: Manhattan, NY 10002 - HYBRID Job

Duration : 3 + months with maximum extension possibility

Shift Time: 9 AM - 5 PM EST Mon - Fri + alt weekends.

Pay Range: $58/hr on W2 - $60/hr on W2 + Stipends (negotiable payrates)

Description:

Guaranteed hours: 37 - 40 hours /week

Field Outreach might be needed.

HYBRID Job

License and Certifications:

NY State RN License is needed.

Schedule Notes:

Bachelor's Degree required; BSN preferred.

Two (2) to three (3) years clinical experience in certified home health agency (CHHA), Lombardi program and/or MLTC or MAP program

9-5 Hybrid

Skills:

5 years minimum of clinical experience as a Registered Nurse.

Leadership experience preferred.

Experience with Care Coordination, Utilization review and discharge planning Computer literacy required.
