Job Summary:
The Care Coordinator assists patient/familieswith psycho social, cultural and financial considerations that may impederecovery. Formulates plan for post-hospitalcare. Demonstrates knowledge of human growth and development,assessments, range of treatment, and care of patients appropriate to the agesof patients served.
The position's tasks and responsibilitiesinclude:
- Youwill be a supportive lead role to peers and Director of Case Management.
- Youwill audit information collection for DMat meeting.
- Youwill participate as a member in the Utilization Review Committee.
- Youwill be a clinical resource for staff.
- Youwill assist with prior authorizations for medications.
- Youwill assist with Observation review list and progressions as indicated.

Qualifications
- Graduateof an accredited nursing school program.
- RNlicense required in an approved compact state.
- 1-3years experience in an acute medical setting.
- Strongclinical assessment skills.
- BLScertification. New employees have up to90 days from date of hire to obtain the American Heart Association BLS forhealthcare providers.
