The Restorative Nurse is responsible for the development, implementation, monitoring and supervision of the restorative nursing program for the facility. The restorative nurse is the liaison between formalized therapy and nursing. The restorative nurse promotes a restorative nursing philosophy among all facility departments.

Essential Job Functions:

  • Plans, develops, implements, evaluates and directs restorative services
  • Completes the required supportive documentation (observations, program quarterly note, care plan, program minute log, etc.) during the 7-day look-back period of OBRA assessments (Admission, Quarterly, Annual, Significant Change in Status) in order to score restorative programming on the MDS
  • Maintains a current list of resident's participating in restorative programs. These lists will be specific as to PROM/AROM, Splint/ Brace, Bed Mobility, Transfer, Walking, Dressing and/or Grooming, Eating and/or Swallowing, Amputation/ Prosthesis Care, Communication, Bladder and /or Bowel Toileting Programs or other restorative programs details
  • Configures documentation in Point of Care for daily documentation of restorative nursing measures for each resident on a restorative program
  • Evaluates and revises restorative programs quarterly and as necessary
  • Develops a comprehensive care plan related to restorative areas including problems, goals and approaches. Insures that all restorative nursing measures are reflected on the care plan. Evaluates care plan quarterly and as necessary.
  • Attends care plan conferences. During care plan conferences report on resident's progress and restorative nursing measures
  • Ensures that the necessary assistive feeding and/or ADL devises are obtained as needed
  • Maintains current listing by resident of all assistive devices and care plan for each
  • Works collaboratively with other departments to ensure the necessary assistive devices are consistently clean, available, addressed on the care plan and removed following discontinuation or discharge
  • When discharged from skilled therapy, ensures that the specific recommendations are implemented
  • Makes daily rounds to oversee and monitor the restorative programs and to provide 1:1 teaching, as necessary
  • Performs or oversees functional observation on admission and quarterly as indicated: A) ADL Functional Analysis B) Joint Mobility Observation C) Bowel and Bladder Observations D) Admission/ Quarterly Restorative Documentation E) Bed Mobility System Observation F) Restraint Observation G) Fall/ Safety Observation H) Any other facility/ state assessments related to restorative needs/ programs a) Motorized Wheelchair Evaluation Form - IL only
  • Oversees Bowel & Bladder programs. Gather information on the use of bowel and bladder appliances, the use of and response to urinary toileting programs, urinary and bowel continence training programs
  • After receipt of 3-day voiding diary completed by CNA, creates an individualized Scheduled Toileting Plan and in-service Nurses and CNA's. Review, evaluates and revises resident response to toileting plan quarterly and as needed. The revision includes individualized time adjustments based on individual resident response
  • Collaborates with Nurses and CNA staff in selecting candidates to be on a bladder training program, assists in setting up the program and overseeing the program to ensure appropriate continuity of care
  • Participates in QA audits for restorative nursing as directed and as needed
  • Ensures that restorative equipment and supplies are available as needed. Ensure that the needed equipment and supplies are in good, clean, working order. Report to Director of Nursing any and all equipment needs promptly
  • Problem solves with staff in regards to issues in providing restorative nursing care
  • Coordinates with staff development to orient new staff relative to roles, responsibilities, principles and practices of restorative nursing
  • Educates and manages the facilities ADL documentation training program.
  • Reviews and assesses appropriate documentation related to ADL activities prior to ARD
  • Reviews and manages the physical restraint reduction program
  • Complete assigned areas of the MDS (G, H, J, K0100, O0500, P)
  • Complete the triggered CAA's for ADL's, incontinence, falls and restraints
  • Continually assesses the need for assistance with ADL's, altered gait/ balance and decreased range of motion
  • Directly supervises employees within the restorative nursing department
  • Ensures care cards are updated following any care plan revision and at a minimum, quarterly
  • Participates in Fall Committee Meeting, update Fall Care Plan as needed, and ensure new assistive device interventions are implemented
  • Monitor Point Click Care (Point of Care) documentation daily and report non-compliance to the DON and/or designee. Complete POC documentation during the ARD period and PRN

Education/Experience:

Must possess, as a minimum, a Nursing Degree from an accredited college or university.

Optional: Certification programs for Restorative Nurses are available through the American Association of Nurse Assessment Coordinators (AANAC) and Pathways etc. The AANAC and Pathways offers credentials such as Resident Assessment Coordinator - Certified and Certified Nurse Executive and Certified Restorative Nurses. In order to become certified, Restorative Nurses must complete a series of on-line courses and/or attend a certification course provided by an approved provider
