POSITION Resident Care Manager
DESCRIPTION Position Summary: The Resident Care Manager will work directly with the Health Services Director and is a key member of an interdisciplinary team that will ensure quality care and delivery of services to the residents living in the community.
Monday-Friday Salary Range $60,000.00- $65,000.00 DOE Essential Duties and Responsibilities: (The following is an outline of responsibilities and not intended to be a comprehensive list)- Develop resident Service Plans and participate in care conferences.
- Input new resident data into Point Click Care and update current resident records.
- Coordinate care with outside providers and maintain appropriate documentation.
- Perform resident evaluations in a timely manner e.g., Pain, Diabetic, Self-Medication, etc.
- Respond to resident/family concerns identified and reported by Caregivers and Med Techs.
- Participate in providing continuing education to staff, as assigned.
- Maintain the Acuity Based Staffing Tool.
- Investigate, evaluate, and document incidents or accidents involving residents and ensure follow-up and update service plan accordingly.
- Work collaboratively with care staff and management team.
- Participate in projects, in-service training and committees as assigned.
- Update ADLs and communicate changes with physicians, care staff and families as needed.
- Follow-up on lab reports and doctor orders, as per policy.
- Other duties as assigned.
POSITION REQUIREMENT Qualifications:- Previous experience in a community based care setting
- Excellent written and verbal communication skills
- Demonstrated leadership skills
- Minimum two years experience in an RCC or RCM role
- Point Click Care experience required
