Registered Nurse Patient Care Coordinator

Full Time 6 Glen Cove Dr. Nursing Day Shift For a limited time MaineHealth is offering up to $10,000 sign on for Registered Nurses with 0-1.99 years of experience and $20,000 for experienced Registered Nurses with 2+ years of experience. Eligible candidates are hired (offer accepted) into a Full or Part time RN position. Bonus amount prorated for Part time hires, per diem hires are ineligible. Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 6 months separation from employment. The RN Patient Care Coordinator uses professional nursing skills to coordinate the health care needs of patients in the practice identified as at risk due to chronic or acute illnesses or those in need of preventative health services. The RN Patient Care Coordinator works with the provider, practice care team, patient, and family to promote self-care management and provides direct care appropriate to the role of the nurse including patient education, motivational interviewing, and development of self-care action plans. Required Minimum Knowledge, Skills, and Abilities (KSAs):
•  Education: N/A License/Certifications: Licensed RN by Maine State Board of Nursing. Experience: Minimum of 2 years' clinical experience preferred. Physician practice or similar ambulatory clinic setting preferred. Strong assessment, goal setting, problem-solving, and decision-making skills. Ability to establish effective working relationships with patients and care team members. Ability to self-motivate, manage conflict, and adapt to change essential. Ability to independently follow established policies, procedures, and care plans.
