The Registered Nurse serves as a leader of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health or supports a peaceful death. The Registered Nurse utilizes best scientific evidence and compassion to assist the patient in navigating their health journey. The Registered Nurse accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures.

Responsibilities:

Essential Functions
• Assessment: Demonstrates refined assessment skills to collaboratively discover the patient's story, integrating the following elements: subjective data, objective data, physical picture, psychosocial components, personal preferences and norms, and learning needs and preferences. Must be able to utilize an ongoing process to interpret data and recognize patterns and trends to determine both immediate and future care needs.
• Planning: Dialogues with the patient/chosen support system to identify what is important to them and incorporate his/her preferences and norms into individualized care . Engages the interdisciplinary care team in planning individualized care. Identifies relevant resources based on necessity and availability. Determines immediate needs and establishes realistic short term and long term goals based on desired outcomes. • Implementation of Interdisciplinary Care: Acts with patient advocacy at the core of nursing care . Collaborates with healthcare team to educate patient and their support system. Analyzes, modifies, and implements strategies to progress the patient through their care. Ensures completion of nursing interventions, medical orders, and other related therapies. Possesses the ability to prioritize job duties according to patients' needs and changing acuity levels. Administers and evaluates medication therapies, performs patient transfers and transports patients via wheelchair, hospital bed, or stretcher to other departments. Ability to lift and carry various equipment including portable cardiac monitors, bedside commodes, etc. Ability to provide routine nursing care such as start IVs, perform dressing changes, etc. for multiple patients in a timely manner. Ability to perform in critical situations including administering CPR.
• Evaluation: Continuously assesses patient's progress toward goals and outcomes. Seeks input from patient and their chosen support system regarding progress toward goals . Analyzes and modifies plan of care with patient and interdisciplinary team as indicated to achieve desired outcomes.
• Communication and Documentation: Communicates individualized care and patient's progress to all members of the care team including the patient and his/her chosen support system. Consistently utilizes appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care. Documents appropriate information in a timely manner.
• Delegation: Appropriately delegates to ensure the implementation of care while ensuring patient safety, considering the nature of the task, competence of the delegate, and the patient's response. Demonstrates commitment to quality patient care by holding self and others accountable for optimal role performance.
• Performance Accountability/Evidence -Based Practice: Demonstrates independence in the foundation of nursing care practice within the specialty. Practice according to your state's Nurse Practice Act, ANA Code of Ethics and ANA Bill of Rights. Seeks involvement in Shared Governance through council participation and/or by staying current with council activities. Identifies opportunities for process improvement, incorporates evidence-based practice and research. Engage in self-reflection and develop personal goals to enhance knowledge, obtain professional certification or pursue academic degree as strategies to promote professional growth. Integrate gained knowledge into practice. Maintain professional portfolio.
• Teamwork: Develops and maintains positive relationships with team members/internal customers. Consistently demonstrates a willingness to adjust activities to accommodate the needs of team members/internal customers. Develops team members through mentoring, precepting, orientation, and providing feedback. Resolves conflicts independently, as appropriate. Accepts responsibility for actions and responds positively upon feedback from others.
• Behavioral Health Specific: Provide behavioral health education to patients and his/her chosen support system, including but not limited to medications , diagnosis, life and coping skills and discharge safety plans/instructions. Knowledge of psychiatric medications, side effects and treatment interventions, knowledge of psychiatric diagnosis, mental health concepts, and evidence based therapeutic interventions and treatments. Ability to establish and maintain control of violent patients, defend self and others against violent assaults, ability to react appropriately in psychiatric crisis, ability to use violent and non-violent restraints per policy and procedure. Ability to comprehend policies and procedures plus ability to lead and influence others and collaborate with others in an interdisciplinary team. Ability to intervene in a therapeutic manner for behavior patterns demonstrated by patients with a mental illness, personality disorder, substance abuse or developmental disabilities.

Other information:

Education: Graduate of an accredited school of nursing required. If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course. 4 Year / Bachelors Degree preferred.

Experience: One year relevant experience preferred

Refer to the Life Support Training Policy NH-HR-3096.

Licensure/Certification: Current RN licensure in appropriate state required. CPI Certification - within three months of hire required.

Additional Skills/Requirements (required) Ability to perform all CPI physical holds. Ability to care for Behavioral Health age specific patients within facility
