Thank you for considering a career at Mercy Health PRN SUMMARY: The Registered Nurse will manage the care provided in the patient's home by planning, coordinating, and providing direct and indirect patient care services in compliance with applicable laws, regulations, and policies of the agency. Must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patient served. KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: 1. Must be a Graduate of an accredited school of nursing. One to two years of recent acute care experience or one year of home health care experience preferred. 2. Required to have a current licensure in the State of Ohio. 3. Possesses and maintains current BCLS certification. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 5. Must have valid driver's license and reliable transportation that is insured in accordance with state and/or agency requirements. PRINCIPAL DUTIES AND RESPONSIBILITIES: 1. Assesses, evaluates and manages the patient's status by reviewing and implementing the plan of care, regularly re-evaluating patient needs, and revising the plan of care as necessary while maintaining appropriate frequency under the physician's direction. 2. Maintains communication regarding patient care with the physicians, ancillary staff and supervisory personnel. 3. Assures that patient satisfaction results meet established goals. 4. Obtains reauthorization for ongoing or additional services. Inputs necessary data in an accurate and timely fashion to reflect utilization review authorization. 5. Assures supervisory visits are completed per Agency and regulatory guidelines. 6. Participates in case conferences with multidisciplinary team members. 7. Assures patients referred to the Agency meet admission criteria and regulatory guidelines. 8. Assures that patient files have required documentation that supports Agency and regulatory guidelines. 9. Completes skilled nursing visits and accepts on call per Agency requirements. 10. Administers medications and treatments as prescribed by the physician. 11. Completes, maintains and submits accurate and relevant clinical documentation regarding patient's condition and care given. Documentation is submitted according to deadlines established and meets agency and regulatory criteria. 12. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician. 13. Provides and maintains a safe environment for the patient by practicing infection control techniques and OSHA guidelines 14. Productivity requirements are met according to established standards. 15. Maintains patient confidentiality. 16. Demonstrates behaviors consistent with stated Mission and Core Values of Mercy Health. 17. Provides service to all age groups. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patient served. 18. Demonstrates knowledge of the principles of growth and development over the life span by completing continuing education program. 19. Interprets information needed to identify patient's requirements relative to his/her age specific need. Neonate Pediatric Adolescent Adult Geriatric 20. Adheres to the standards and policies of the Corporate Responsibility Program, including the duty to comply with applicable laws and regulations, and reporting to designated Manager (or employer hotline) any suspected unethical, fraudulent, or unlawful acts or practices. 22. Adheres to all standards and policies regarding safety/patient safety initiatives. 23. Is responsible for all other duties as assigned. WORKING CONDITIONS: May require stair climbing. Requires ability to independently lift, turn, and position patients. Exposure to communicable diseases, toxic substances, bodily fluids, and medicine preparations. Exposure to dirt, dogs, and other household pets. Exposure to temperature extremes, inclement weather and various socioeconomics environments. Requires tolerance of a stressful work environment.
