The Home Care RN provides to patients in the home setting utilizing the nursing process of assessment, planning, implementation and evaluation of safe and therapeutic care. Responsibilities:
• Ensures quality nursing care is rendered to all.
• Utilizes the Nursing Process, plans, directs and supervises the care of patients in addition to being involved in the provision of direct care of patients and families.
• Develops a written plan of care that guides nursing intervention and facilitates desired outcomes.
• Implements interventions based on the plan of care in accordance with client and caregiver level of skill and knowledge.
• Evaluates client/ family responses to interventions and changes the plan as required.
• Document care provided and client/ family responses consistent to agency policy and procedure.
• Follows agency policies and procedures.
• Communicates with physician and other disciplines regarding client status. Coordinates services as needed.
• Utilizes community resources in the development of plan of care.
• Supervises ancillary personnel (Home Health Aide, Shift Professional Nurse) assigned to client care.
• Attends educational programs to maintain current standards of practice.
• Responsible for employee injury prevention, follows safety rules, and addresses safety issues with supervisor.
Qualifications:
• Must be graduates from an accredited School of Nursing with current NYS RN license.
• 2 years of nursing experience preferred.
• Home Care experience preferred.
• Strong verbal and written communication are necessary and basic computer skills are preferred.
• Must possess valid NYS driver's license and current automobile insurance and able to drive during work hours.
