Summary of Position: The Registered Nurse (RN) works with all members of the Patient-Centered Medical Home (PCMH) Care Team by performing a variety of clinical and administrative patient-related duties as established in the roles and responsibilities defined below. The RN Actively participates in team huddles and care team meetings. The RN provides professional nursing care, that is safe and effective, utilizing established policies and procedures. The RN assesses, plans, implements and evaluates care in collaboration with physicians and practitioners. The RN provides leadership and supervision to all clinical staff. The RN coordinates department activities, ensuring safety of patients and staff and ensures that adequate medical supplies, medication and vaccinations are ordered and are on the premises. Services are provided based on the ages of the population served, newborn through geriatric (65 years or older), in collaboration with the care team. Qualifications Education: Must be a graduate from an accredited school of nursing with a current Registered Nurse license specific to the State of employment Bachelor's degree in Nursing (BSN) preferred. Experience: At least two years of nursing work experience. Ambulatory care/physician office experience preferred. Prior experience with chronic disease management and PCMH preferred. If applying for UC/Occupation Health Services o Emergency room, or Urgent Care experience is preferred Knowledge/Skills/Abilities: Duties require intensive knowledge of a highly specialized field or broad knowledge of major hospital and PCMH functions or activities. Extensive knowledge of epidemiological principles, infectious / communicable diseases, health assessment, health management, and adult immunizations. Highly effective written and verbal communications skills. Excellent interpersonal skills. Proficient computer skills as required to document, communicate and enter information into the electronic medical records system. In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred. Certifications/Licensure: Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date. If applying for UC/OHS position a current ACLS card will be required or must be able to complete an approved outside course, or the first available Barton University certification course. Valid state issued driver's license. Will use own vehicle to travel to off-site locations; need proof of car insurance and registration. Current California Nursing License in good standing for California position. If applying for Nevada position current California and Nevada Nursing licenses, in good standing will be required. Occasional travel to various health system locations. Essential Functions 1. Provides consistently exceptional care at all times. 2. Provides and educates patients on abnormal results per physician/provider direction. 3. Under the direction of the medical provider, perform diversified duties both within and in support of the department. Assists physicians/practitioners in providing medical care, performing procedures including participation in codes. 4. Assists physicians with examinations, procedures and other processes related to direct patient care. Keeps patient/family informed. Focuses on customer service needs. 5. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served. The individual demonstrates knowledge of the principles of growth and development over the lifespan and possesses the ability to identify each patient's age specific needs. 6. Meets with injured/ill patients as required or needed involving: a. Triage of the individual's injury/illness. b. Facilitate processing and treatment of involving employee working through and in cooperation with all designated treating facilities. c. Work in cooperation with the applicable treating physician. d. Establish plans of care and rehabilitation for the injured/ill patient to return to work. e. Instruct individuals in carrying out physician discharge instructions and the related activities including (for injured patients) the placing of calls to various client companies regarding injured employees pertaining to injury itself; plan of care; estimated lost work time, etc. f. Prepare written reports regarding injured workers and fax/mail/deliver to applicable area client company representative. g. Disease specific management 7. Monitors and ensures accuracy of documentation in patient medical record. All aspects of patient care are accurately and completely documented in appropriate medical record system. 8. Provides supportive and compassionate leadership to clinical staff. a. Responds to staff requests in a timely manner. b. Ensures accurate and timely communication. c. Is supportive of staff when dealing with conflicts involving other disciplines and services. d. Approach is non-judgmental and professional. 9. Coordinates clinical activities of the department, ensuring safety of patients and staff. 10. Models professional communication with other disciplines and departments. Ensures the competency of clinical staff. Oversee training and ensures clinical staff competencies, ensures safe clinical staffing levels. Supervises, trains and evaluates work performance of all clinical personnel. 11. Assesses pain level on all patients upon initial contact and reassesses according to standards. a. Evaluates location, severity, intensity and quality of pain. b. Uses the approved pain scale for measurement c. Reevaluates pain level in response to intervention/ treatment 12. Assesses psychosocial needs of patients, expressing interest in their progress and comfort. 13. Provides telephone advice completing telephone triage and callbacks, assign tasks, related reports and follow through of physician instructions in a timely manner. 14. Confirms medication administration is safe, accurate and timely. a. Follows all safety precautions related to patient ID, order confirmation, calculations and double-checking dose accuracy if applicable. b. Follows procedure when removing stock meds. c. Signs and ensures all narcotic waste is witnessed. d. Documents all medication intervention to include rationale. e. Reports all Adverse Drug Events (ADE), Adverse Drug Reaction (ADR) and medication errors, according to policy. 15. Ensures vaccination administration is recorded in appropriate state reporting system 1. Recognizes unusual conditions outside of his/her expertise and seeks guidance of coworkers or supervisors. 16. Performs nursing care essential to the assessment, promotion, maintenance and restoration of patient's health and wellness. 17. Ensures daily quality controls such as, but not limited to: Clintek, POC testing, lab controls, hemoglobin equipment, refrigerator temperature, emergency equipment and medications, narcotics and water temperature. 19. When appropriate: Effectively educate patients and families on: a. Medication use and safety b. Nutrition interventions c. Safe use of medical equipment d. Skin and wound care management e. Management of pain f. Rehabilitation techniques g. Use of other resources h. Chronic disease management 20. Operates equipment and performs job related duties in a safe manner. Insures proper functioning of equipment and follows procedure when equipment malfunctions. 21. Develops and updates policies and procedures; able to access documents and adheres to all policy and procedures that guide the delivery of care. 22. Under direction, proposes, develops and presents a variety of health education programs to the local area general public and/or area client companies. 23. Performs diversified duties regarding pre-placement physical examinations. Coordinate all applicable testing with technicians.
