JOB DESCRIPTION Summary/Objective Assists medical professionals by providing nursing assessments, identifying care requirements, and monitoring psychobiological treatment regimens in a Sub-Acute residential setting to stabilize mental well-being and coordinate treatment or aftercare services as needed. Essential Functions Provide nursing assessment within 24 hours of admission Administration of medication and medication management as ordered and prescribed by the provider Provide education regarding side effects and assess the effectiveness of medication. Notifies physician of side effects or other concerns and/or lack of effectiveness. Arranges medication refills, as ordered and prescribed. Monitors lab reports and relays results to the physician, client, and therapist. Monitors vital signs, as ordered by the physician. Collaborate with the on-call therapist, in conjunction with the medical provider. Coordinates with other Volunteers of America staff and other community providers in collaborative treatment. Provides program and case consultation and education services internally, as well as to other community groups and individuals. Promotes wellness and provides education about health issues. Develops and maintains a positive working relationship with other community providers. Prepares needed reports of treatment as requested. Maintains case records to reflect the course of treatment. Adheres to VOA policies and procedures regarding proper documentation in the EHR and Dr's First. Provides follow-up and aftercare planning services as needed. Participates in agency program development activities Attends staff meetings, supervisory conferences, and other activities as assigned by supervisory staff. Competencies Ability to effectively work with a variety of computer software programs, including Microsoft Word, PowerPoint, and Desktop Publishing Effective communication skills, both oral and written. Excellent attention to detail and organizational skills Successfully interact with clients and team members in a professional manner Ability to prioritize and manage multiple tasks concurrently Demonstrate independent work initiative and sound judgment Continuous demonstration of behaviors aligned with a posture of servant leadership and our cultural values Supervisory Responsibility This position has no direct supervisory responsibilities but may serve as a coach and mentor for other positions in the organization. Work Environment This is an on-call position in a 24/7 facility. Days and hours of work vary according to the assigned shift. This role routinely uses standard office equipment such as laptop computers, photocopiers, and smartphones. Physical Demands The physical demands described here represent those that an employee must meet to perform the essential functions of this job successfully. While performing the duties of this job, the employee is regularly required to talk or hear. Specific vision abilities this job requires include close vision, the ability to adjust focus, and the ability to lift files, open filing cabinets, and bend or stand on a stool as necessary. Position Type/Expected Hours of Work This is a full-time position. Days and hours of work are generally Monday through Friday, 8:00 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties and on-call demand. Travel Some travel is required for this position.
