Registered Nurse (RN) - Home Health - Part Time Contract

We are seeking a Contract Registered Nurse (RN) to support a Home Health Care client. You will play a crucial role in providing personalized care and support to individuals in the comfort of their own homes.
  • Expectations are to see a patient for 1-hour sessions (3) times per week. More hours will be available by accepting additional patients.
  • Part Time Workers will get 3 to 12 hours per week depending on number of patients seen.

POSITION: Contract
PAY RATE: $25.00hr
INTERVIEW: Will be in Plano, TX

Responsibilities:
  • Conduct comprehensive assessments of patients' health status, including physical, emotional, and psychosocial needs, and develop individualized care plans in collaboration with the patient, family, and healthcare team.
  • Provide skilled nursing care, including wound care, medication management, intravenous therapy, and patient education, following evidence-based practice guidelines and regulatory standards.
  • Monitor patients' vital signs, symptoms, and responses to treatment, and modify care plans as needed to optimize outcomes and promote patient safety and comfort.
  • Coordinate with physicians, therapists, social workers, and other healthcare professionals to ensure interdisciplinary collaboration and continuity of care for patients.
  • Perform skilled nursing procedures and interventions as prescribed, including but not limited to catheter care, tracheostomy care, ventilator management, and ostomy care.
  • Educate patients and their families about disease management, medication administration, self-care techniques, and community resources to promote independence and empower patients to manage their health effectively.
  • Assess the home environment for safety hazards and provide recommendations for modifications or adaptations to support patient safety and mobility.
  • Document all patient care activities, assessments, interventions, and outcomes accurately and in a timely manner, maintaining confidentiality and compliance with regulatory standards.
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care, outcomes, and satisfaction.
  • Adhere to ethical and professional standards of nursing practice and conduct, demonstrating empathy, respect, and cultural competence in all interactions with patients, families, and colleagues.

Qualifications:
  • Current Registered Nurse (RN) in the state of Texas
  • Minimum of two years of clinical nursing experience, preferably in home health care or a related setting.
  • CPR and BLS certification required.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills, with the ability to establish rapport with patients and families.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in electronic medical record (EMR) documentation and computer skills.
  • Reliable transportation and valid driver's license.
  • Ability to pass a criminal background check and drug screening.
