H2 Homecare is searching for nurses who are dedicated to providing excellent care for patients located throughout Georgia. We support our nurses through on-site training with our clinical supervisors, as well as a supportive team that will work with you to provide flexible hours, a supportive orientation, and training on our EMR (electronic medical record) system.

Credentials:
  • Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the State of Georgia
  • Graduate of an accredited practical nurse or vocational nursing program
  • One year of prior professional nursing experience, homecare experience preferred
  • Current CPR certification
  • Clean background check
Job responsibilities:
  • Changes in the patient's condition are identified and documented with resolution on the appropriate clinical form
  • Physician notification to obtain necessary orders for intervention(s) per state regulations
  • Performs specific treatments and medication administration in accordance with physician orders
  • Documentation reflects patient/family collaboration and participation in their plan of care
  • Documentation and clocking in and out utilizing the current patient health record platform, Alayacare
  • Skilled observations and significant changes in patient status/care are communicated to the Director of Clinical Services and/or Clinical Supervisor/Case Manager
  • Request for modification of orders and additional orders are made based on the patient's clinical status and Skilled nursing judgment
  • All physician collaboration /coordination of care is documented in the medical record and communicated as needed
  • Adheres to H-squared Homecare policies and procedures
