The Cleft & Craniofacial Nurse Coordinator works with fellow team coordinators to facilitate the function and efficiency of the team, ensuring coordinated care for patients and families. This role is responsible for assisting families in understanding, coordinating, and implementing treatment plans. Responsibilities include ensuring that providers and patients are coordinated appropriately for each cleft and craniofacial team visit. This position will prepare patient records for each patient's cleft team visit, which includes requesting reports, test results, or other pertinent information from community providers to ensure care is coordinated and comprehensive. On clinic day, this position will assist with managing clinic flow, answering family questions, and serving as a resource for fellow team members. The coordinator will assist with leading the team conference, making sure to document and understand the recommendations and overall treatment plan for each patient. This position will follow up on patients after clinic to review the team's recommendations, ensure parental understanding of the treatment plan, offer post-clinic counseling, provide educational information and arrange referrals as needed. Other duties include managing incoming clinical phone triage; creating educational materials for patients and families; and assisting with coordinating educational opportunities for team members, families, and the community. This role will be responsible for helping orient new team members, assisting with data collection and supporting research projects for the team. The nurse in this position will act as a role model, resource person and member of the leadership team, as well as keeping the organization's mission and vision at the forefront of all patient care and system activities. Requires an Associates of Nursing (ASN). Bachelors of Nursing (BSN) preferred. Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. Degree from a nursing program accredited through the ACEN or CCNE with a current Indiana Registered Nurse License. Two years experience in an ambulatory setting preferred. Knowledge of procedures for performing the functions appropriate to the specific clinical area and familiarity with designated pieces of equipment and supplies. Requires ability to coordinate a number of functions for a group of patients in an effective timely manner. Requires ability to assess patients without face-to-face interaction. Requires awareness and understanding of legal implications of patient care activities and performance as a Registered Nurse.
