Job Description:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The RN will be responsible for providing skilled nursing care to patients in their homes, ensuring high-quality and personalized care tailored to each individual's needs. This role requires strong clinical skills, excellent communication abilities, and a commitment to delivering exceptional patient-centered care.
Duties:
- Provide skilled nursing care to patients in their homes
- Conduct initial assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patients' conditions and report any changes to the healthcare team
Requirements :
- Must be a licensed Registered Nurse (RN)
- Previous experience in home health or a related field is preferred
- Proficient in medication administration, including IV therapy and tube feeding
- Familiarity with acute pain management techniques
- Ability to work independently and make sound clinical judgments
- Strong communication and interpersonal skills
