Job Summary Reporting to the RN, Manager Prisma Health SeniorCare PACE, the Homecare Supervisor is responsible for the development and implementation of homecare services for program participants. Includes the coordination of nursing, PT, OT and ST services as well as Durable Medical Equipment in the community and nursing home. Accountabilities "Prisma Health SeniorCare PACE is an outpatient setting. Schedule is Monday through Friday with daytime hours. Prisma Health SeniorCare PACE offices are closed on holidays and weekends." This position is for our White Rock / Chapin Location Coordination of 24 hour care delivery and the implementation of all homecare services which includes nursing, PT, OT, ST and personal care services to ensure that quality services are provided to meet participant needs. - 20% Assesses the homecare needs of a frail elderly population, and identifies and develops specific plans of care. Participates as a member of the Interdisciplinary Team, attends morning meeting and communicates any changes in participant condition to team members. - 20% Coordinates and authorizes all Durable Medical Equipment. - 20% Coordinates and supervises all nursing care for participants in the nursing home/boarding home according to program standards. Includes interaction with other facility staff. - 20% Supervises assigned home care staff and is responsible for scheduling, performance review, evaluations and related human resource duties for direct reports. - 20% Supervisory/Management Responsibility Job has direct and/or indirect supervision of team members that may include final budget authority, hire/termination authority, performance appraisal responsibility and disciplinary authority. Job will be considered a member of management staff at Prisma Health Company or affiliate and will have direct reports. Minimum Requirements Bachelors Degree - Nursing 1 year - at least one year of nursing experience and one year of experience with the frail or elderly population Required Certifications/Registrations/Licenses Licensed RN in SC. Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.
