Job Details:Two Post Heart Transplant Coordinators triage all calls, pages, Patient Gateway Messages providing open communication with patients during the week. Reviewing labs, medication orders, and collaborating with MD/NP groups on patient needs in a timely manner. Division of labor of patient population by Alphabet, ex. All patient with last name starting with A-G. No Oncall, No weekends, No Holidays. 4 days a week, onsite requirement 3 days a week. RN Clinical Experience: Ambulatory coordinator experience - procedure knowledge/results review/nurse triage/medication administration/EPIC
Requirements:Active MA license 2 years exp Covid vaccine & booster Flu shot Post Transplant Coordinator experience
