Primary Responsibilities/Accountabilities:
- The RN Case Manager is an expert professional registered nurse who is responsible for developing and managing patient care outcomes for an entire patient case load.
- The RN Case Manager may perform assessment, treatment or care for patients of all ages, including neonate, pediatric, adolescent, adult and geriatric, based on population focus.
- Duties also include complex discharge planning and utilization review.
- The Nurse Case Manager must be competent to analyze patient data with an understanding of the cognitive, physical, emotional/psychosocial, spiritual, and chronological maturation process.
- The Nurse Case Manager is accountable for meeting clinical and financial outcomes for the patient within an appropriate length of stay, with the effective use of resources and within established quality standards.
- The Nurse Case Manager has the authority to direct care and collaborates with the patient/family, assigned nurse, and the health care providers to accomplish these outcomes.

Bachelor's Degree in Nursing.

1 year home health, mental health, substance use, hospice, and/or utilization review.

Progressive nursing leadership experience, including charge nurse role.
