Hours/day: 8hrs Day
Contract Duration:13 weeks
License Requirement: Oregon RN license or pending accepted
Certification Requirement: BLS

Job Title: Registered Nurse (RN) - Case Manager

Job Description:

As an RN Case Manager, you will play a crucial role in coordinating and overseeing the delivery of healthcare services to our patients. The primary focus will be on developing and implementing care plans, collaborating with healthcare professionals, and ensuring the continuity of care across various healthcare settings. The successful candidate will demonstrate strong organizational skills, clinical expertise, and a commitment to providing patient-centered care.

Key Responsibilities:

  1. Comprehensive Assessment:

    • Conduct thorough assessments of patients' medical, social, and psychological needs to develop a holistic understanding of their health status.
    • Collaborate with healthcare team members to gather relevant information and contribute to the development of individualized care plans.
  2. Care Planning and Coordination:

    • Develop and implement comprehensive care plans based on assessment findings, ensuring they align with the patient's goals and preferences.
    • Coordinate and facilitate communication between healthcare providers, specialists, and support services to ensure seamless and effective care delivery.
  3. Patient Advocacy:

    • Serve as an advocate for patients, ensuring their needs and preferences are communicated and addressed throughout the care continuum.
    • Collaborate with patients and their families to promote shared decision-making and active participation in their care.
  4. Utilization Review and Resource Management:

    • Monitor and evaluate the appropriateness of healthcare services provided, including the utilization of resources and adherence to established guidelines.
    • Identify and address potential barriers to care and implement strategies to optimize resource utilization.
  5. Communication and Documentation:

    • Maintain accurate and timely documentation of patient assessments, care plans, and interventions in electronic health records (EHR).
    • Communicate regularly with healthcare team members, patients, and families to provide updates on care plans and address any concerns.
  6. Quality Improvement:

    • Participate in quality assurance activities and contribute to the development and implementation of quality improvement initiatives.
    • Stay abreast of industry best practices and evidence-based guidelines to enhance the quality of care provided.
