Key Areas of Responsibility - Ensure accuracy of all sections of Multiple Data Set ( MDS ) coding to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care ( CMI ).
- Ensure accuracy of clinical paperwork (assessment and accompanying documentation) to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care.
- Updates Medicare sheet every morning.
- Completes short and long-term MDS.
- Insurance updates.
- Look through hospital records for all new admissions.
- Must be certified with AAPACN (American Association of Post Acute Care Nursing) with more than 3 years of experience.
- Must know how to obtain a CMI (Case Mix Index) measures.
- Care plan proficient and provide care plan education to the nursing team including skilled notes CNA documentation.
Additional Duties and Responsibilities: - Case management w/ managed care (maximize therapy and nursing services provided to residents in order to maximize patient-centered outcomes)
- This position manages clinical and financial record keeping and dissemination of these records to appropriate payors. This includes overseeing UMR reviews and any appeals processes.
- Take joint ownership of care conferences with social workers.
Reporting structure - Reports directly to DoN
- Surrogate for DoN in managing Nurse Managers
