Type:Full TimeSalary/Pay Rate:$48,000 - $51,000Posted Date:02/20/2024 1:02 PM Richard Hall Community Health and Wellness Center 37.5 Hours/week; Full-Time; Exempt; Grant-Funded Responsible for working with individuals in the STAR Program to support and strengthen their capacity to engage in their personal recovery from OUD and co-occurring problems. Will help individuals with their most pressing early recovery needs in the four dimensions that support a life in recovery: home, health, purpose and community to remain in the Recovery Zone. Essential Functions Provide recovery support and peer coaching to the STAR program participants. Develop a measurable and strength-based Recovery Plan in collaboration with the individual. Educate how to navigate treatment, social services and recovery support systems. Provide recovery support services based on the individual's preference and needs. Support the individuals as they move through the stages of change to encourage them in their recovery. Assist with accessing recovery support services in the community. Work collaboratively with the case manager to ensure the individual engages in services for up to one (1) year. Be a positive role model by sharing experiential knowledge, hope and skills. Maintain relationships with the individual in order to assist him/her in the treatment engagement and retention process. Reinforce, guide and reassure the individual that recovery is possible, and is built on multiple strengths, coping abilities and the resources of each individual. Assist the individual with gaining skills and resources needed to initiate and maintain recovery and maintain compliance with MAT. Assist in establishing and sustaining a social and physical environment supportive of recovery. Enhance identification and participation in the recovery community. Advocate for appropriate and effective community treatment and recovery. Empower individuals to make self-determined and self-directed choices about their recovery pathway. Provide support with face-to-face sessions and/or telephone support based on the individual's personal choice. Maintain follow-up one year after starting the STAR program. Help individuals maintain healthy community, family and social functioning. Essential Qualifications High school diploma or equivalency. Two (2) years' experience in the guiding principles of recovery that assist individuals to improve their health and wellness, live a self-directed life, and reach their full potential. Two (2) years' experience in providing services to individuals diagnosed with OUD and their families/caregivers who have co-occurring substance use and mental health disorders, including LGBT; pregnant women; IV drug users; justice involved; veterans; individuals with complex trauma and those exposed to adverse childhood experiences who are at risk of developing chronic diseases due to health disparities. Has experience in working across cultures. Lived experience in mental health and/or substance use disorders in recovery for a minimum of five years. Must be computer proficient and possess a valid NJ driver's license. Preferred Associate's degree or above. Bilingual (English/Spanish), knowledge of Wellness and Recovery Principles, Person Centered Care, Trauma Informed Care and Integrated Healthcare.
