Required Upon Submission: Minimum Years Clinical Experience Required: 1 yr.

Minimum Years Clinical Experience in Specialty Required: 1 yr.

Certifications Required: ARRT, BLS, ACLS

Prior Travel Experience Required: No

No WH gaps over 30 days w/o explanation

Must have Trauma Level, EMR, and No. of Beds filled out

2 References- dated no more than 12 months old.

Skills Checklist - dated no more than 12 months old.

All RTO must be in packet at time of submission.
