Requirements

This position will carry out the following responsibilities:
  1. Regarding infectious disease:
    1. Complete case investigations within the Utah Department of Health statewide database and the Navajo Nation database;
    2. Complete contact tracing for infectious disease investigations;
    3. Provide patient education for treatment, transmission, and prevention of infectious disease;
    4. Participate as needed in vaccine clinics, mobile testing, and other community outreach activities;
  2. Regarding the home visiting program (as available according to grant funding):
    1. Obtain an annual federal background check, according to the requirements of the Office of Home Visiting (OHV);
    2. Recruit families, according to need, to the home visiting program;
    3. Perform regular home visits according to program guidelines;
      1. Perform family assessments;
      2. Determine needs;
      3. Perform developmental assessments;
      4. Provide teaching and handout materials;
      5. Participate in regular group activities according to program guidelines;
  3. Regarding home visits to patients not enrolled in the home visiting program:
    1. Accept assignments, according to time and availability, from case manager for welfare checks to patients recently discharged from hospital:
      1. Prepare for welfare check by accessing discharge documents from the patient's electronic medical record;
      2. Provide education to patient and family regarding discharge instructions to ensure patient and family understanding;
      3. Obtain vital signs as necessary, such as spO2;
      4. Ensure that patient and family have contact information for additional resources such as behavioral health, medical supplies, follow-up, etc.
      5. Make referrals, if necessary, to the behavioral health team, community resources, etc.
      6. Document all actions, interventions, and referrals in the patient's electronic medical record;
    2. Accept assignments, according to time and availability, from the provider for nursing care, welfare check, medication delivery/administration, etc.
      1. Ensure provider's orders are in the chart before performing home visit;
        1. May enter provider's orders into the patient's chart if requested;
      2. Perform action according to provider's order;
      3. Document action into patient's electronic medical record;
        1. Communicate with provider via secure AthenaText;
        2. Complete the order;
  4. Regarding health fairs, community health events, etc.:
    1. Prepare for the event with handouts, posters, giveaways, etc. relevant to public health;
    2. Participate in the event at all four clinic sites, and as directed by supervisor;
    3. Coordinate and network with local agencies to provide health-related activities to promote awareness and provide education;
  5. Regarding trainings, workshops, and continuing education:
    1. Attend relevant training for issues related to public health, such as: diabetes, healthy pregnancy, maternal mental health, Parents As Teachers, Family Spirit, or other home visiting program, childhood development, immunizations, sexual health, etc.;
  6. Any other tasks, projects, or duties requested by the Employee's supervisor or other UNHS management or administration that is reasonably related to UNHS and/or the Employee's employment with UNHS. Such tasks may include:
    1. Assist in the completion of special studies, projects, or research programs.

Education & Experience :
  • Utah State Licensed Practical Nurse or Registered Nurse Licensure.

As a Tribal Organization the Utah Navajo Health System, Inc. (UNHS) treats patients with high risk and underlying chronic medical conditions. Therefore, UNHS requires its employees to show proof of immunization prior to their employment with UNHS or during their employment. Below is a list of immunizations/vaccinations required to be employed or to continue employment with UNHS.
The immunizations/vaccinations include but not limited to following:
  • MMR (Measles Rubeola, Mumps, Rubella): Documentation of two MMR vaccines (OR) Documentation of MMR titers.
  • Complete Hepatitis B vaccine series (3 doses), documented proof of titers indicating immunity, or a declination that may be signed upon arrival.
  • Proof of up-to-date PPD skin testing, if previously negative (OR) Proof of positive PPD skin test with the most recent chest x-ray and treatment history.
  • Tetanus, diphtheria, and pertussis (Tdap).
  • Varicella titers, history of varicella or varicella immunization series.
  • Influenza vaccine for the current year.
  • COVID-19 vaccine
  • Any other vaccinations as requested.
