You will besupported by an entire care team, including a health coach, to deliver carethat is often outsourced to specialists in today's broken system. Compensation: $140.00 pr/hr
Education & Experience:
- MD or DO degree
- Completion of a Family Medicine orInternal Medicine Residency, Family Medicine preferred
- Current Board Certification by ABMS(ABFM/ABIM) or AOA
- Experience working in EMR systems,NextGen preferred
- Experience in coding anddocumentation, including Medicare Advantage and HCC

Essential Functions:
- Provide evidence-based primary care to10 to 15 patients per day,
- Focusing on prevention, deliveringpractical treatment for chronic and acute care needs, and providing disease andcase management services
- Partner with the patient and your careteam, including an onsite Whole Health Coach, to develop effectivebiopsychosocial patient care plans that meet the needs of each unique patient,respecting their diverse backgrounds
- Ensure comprehensive care in alignmentwith the medical home model, including collection of comprehensive healthhistory, performing appropriate physical examinations, conducting diagnostictesting, and prescribing medications - all with the convenience of onsite laband pharmacy
- Assess patient population needs andwork with team members to drive participation in annual whole healthevaluations, health coaching, and other health promotion activities
- Provide clinical leadership, includingparticipation in team meetings, clear proactive communication, regularprioritization of key efforts, process improvement, and actively supportingchange management Rotate with other providers in the region to ensure coverageafter-hours
- Participate in organizationalactivities and committees (e.g. weekly provider meetings, Clinical AdvisoryCommittee meetings, Quality Improvement Committee, etc.) to optimize processesand programs
