Qualifications:

Successful completion of a Certified Nursing Assistant (CNA) course approved by the State of Illinois - preferably on the Illinois Health Care Worker Registry

Criminal Background Check with no disqualifying conditions, DMV records showing a good driving record

Reliable transportation, valid driver's license, and current auto insurance.

Current/Continuous CPR and Basic First Aid certifications.

Exhibits a positive attitude and demonstrates leadership qualities and effective interpersonal relationships with both internal and external customers.

Can work independently with minimal supervision.

Ability to lift 50 lbs.

Need to be able to stand, crouch or bend for long periods of time.

Covid Vaccination - Preferred, but not required.

Job Summary:

The Certified Nursing Assistant is a certified paraprofessional specifically orientated to assist a Private Duty Home Service client with activities of daily living in their place of residence, as specified in the individuals Plan of Care, with adherence to At Home Quality Care's standards. Duties are limited to those in which the CNA has demonstrated competency. The CNA for Home services is directly supervised by the Private Duty Coordinator as an employee of At Home Quality Care.

Job Responsibilities:

Observation of client functioning and environment status and reporting changes to the supervisor both orally and in documentation.

Carry out tasks as outlined in instructions in each individual clients Plan of Service of Plan of Care.

Provide assistance with incidental household chores including initiating proper diet, meal preparation, light housekeeping, laundry and linens for client.

Work with clients, family, and agency to create and maintain a safe environment and report any hazards.

Assist client with activities outside the residence, including shopping, errands, and appointments.

Provide companionship, socialization and diversional activities including mental stimulation and emotional support.

Assist with activities of daily living and personal care with the scope of services specified by all deferral and state licensing bodies and according to agency policies, including: bathing, routine hair care, dressing, foot and nail care, routine skin care, shaving, oral hygiene, eliminations, nutrition, feeding, positioning, safe transferring and ambulation, exercise and medication reminding or prompting.

Initiate CPR, Basic First Aid, and contact EMS in case of emergency.

Maintain compliance with agency policy and procedures regarding infection control.
