Legacy is recruiting a Nurse Practitioner for our Woodburn Health Center Clinic located at 1475 Mt Hood Ave Suite 120, Woodburn, OR. Our clinics support local communities by providing services to a variety of patient populations. With more than 25 clinics serving the Portland/SW Washington surrounding communities, and rural locations such as Silverton, Keizer, Sandy and St. Helens, Legacy is helping keep the Pacific Northwest healthy. Our clinics have well-supported teams to include panel coordinator, social worker, RN case manager, behaviorist, pharmacist as well as an individually assigned MA. Responsibilities Responsible for the patient throughout the course of treatment, including reviewing clinical history, providing the physical examination, making the diagnosis, and treating both chronic and episodic disorders. Work in collaboration with the team and seek the advice of a team physician whenever a case falls outside the scope of expertise. Maintain own panel of patients and share call coverage for the clinic. Qualification and Licensure Requirements. National certification as a Nurse Practitioner (NP) required. Current applicable state license as a Nurse Practitioner with Prescriptive Privileges (NP-PP) required. Unrestricted Washington Medical license.
