Position Summary The Registered Nurse (RN) provides and coordinates the care of patients/families and significant others in the ambulatory care setting by assessing, planning and intervening in actual or potential health problems, including evaluation of the patients response to the care. These responsibilities also include patient/family/significant other education and health maintenance strategies. The RN is a member of a designated health care team and collaborates, coordinates and guides team members in planning patient care and achieving goals. The Registered Nurse demonstrates knowledge of the principles of human growth and development over the life span and the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his or her age, specific needs, and to provide the care needed as described by standards and policy and procedures. Essential Functions and Responsibilities Includes the Following: 1. Understands and adheres to the WPH Performance Standards, Policies and Behaviors. 2. Demonstrates the ability to perform clinical/technical/service/administrative tasks 3. Systematically and continuously assesses patient physical, psychosocial and spiritual health status and knowledge base to determine patients health care and educational needs. 4. Provides and/or delegates patient care according to the care goals. Sets priorities based on patient need. 5. Triage all sick and walk-in patients 6. Establish therapeutic and supportive relationships with patients and family. 7. Acts as a resource person. 8. Coordinate client care including referral back to primary provider. 9. Assess needs and initiate referrals (VNS, Social Work). 10. Telephone triage. 11. Prescription requests, according to procedure. 12. Communicates normal and abnormal test results in collaboration with provider including appropriate followup. 13. Performs therapeutic treatments and procedures. 14. Assist with procedures. 15. Administer oral/IM/IV/SQ/Inhaled medications. 16. Coordinates delegates and/or provides all other patient care activities. 17. Coordinates in-patient admissions and ER referrals. 18. Acts as a patient advocate. 19. Documents according to standards of care. 20. Promotes and provides for patient safety in accordance with state and federal guidelines. 21. Participates in the departments performance improvement activities. 22. Provides presentations to care team members. 23. Ensures all staff participates as a member of the patient care team, participates in daily huddles and attend regular scheduled practice team meetings. 24. Attends in service and continuing education programs. 25. Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others. 26. Informs and educates patient/family to enable participation in their care and decision-making. 27. Evaluates the effectiveness of interventions and patient responses and outcomes to care. 28. Collaborates with team members to make the necessary changes to patient plan of care. 29. Communicates pertinent information and changes in patient status to appropriate members of the care team and other health care providers. 30. Performs all other related duties as assigned. Education Experience Requirements 1. BSN required. 2. Graduate from an accredited school of nursing with current New York RN registration. 3. BLS certification required (must be through the American Heart Association). 4. ACLS required (as of 6/1/21) for all RNs working in the Cancer Center or Center for Advanced Medicine Surgery (CAMS) 5. Must be able to remain stationary for extended/prolonged period ï' Frequently moves/transports objects up to fifty (50) pounds and must be able to move patients of all weights with a minimum of one (1) assist or assistive device/equipment ï' Must be able to work in environment with continuous interruptions ï' Must be able to assess and respond to rapidly changing situational needs of patients, department and hospital ï' Must be able to adapt in a high stress and fast-paced environment with multiple interruptions ï' Must be able to detect sounds and respond to emergency sounds/signals at all times ï' Must be able to constantly operate computer, equipment and other devices ï' Position requires ability to concentrate on fine detail with interruptions and attend to tasks for more than sixty (60)minutes at a time. ï' Individual must be able to understand and relate to theories behind several related concepts and remember multiple tasks given during the course of a day ï' May be exposed to chemicals necessary to perform required tasks. Any hazardous chemicals the employee may be exposed to are listed in the hospitals SDS (Safety Data Sheet) data base and may be accessed through the hospitals Intranet site (Employee Tools/SDS Access).
