Job Opening: Population Health Administrator Department DCHD Department of Health Division Population Health Posting Date: January 26, 2024 Salary: $95,004.00 / Annually Shift: 8:30am - 4:30pm Monday - Friday / 37.5 Hours Summary The Administrator of Population Health of the Delaware County Health Department (DCHD) is responsible for leading the Division of Population Health. Responsibilities include planning, organizing, and administering population health initiatives including public health emergency preparedness, response and mitigation of emergency responses, prevention of chronic conditions, promotion of health equity, management of the Delaware County Health Department Wellness Line Call Center, and leading DCHD's collaboration with community partners, agencies, and healthcare institutions to address social determinants of health that impact individual and community health. The Administrator of Population Health leads the development of public health policy and implementation of Delaware County's Community Health Improvement Plan for DCHD. This position uses public health data across Delaware County to develop strategies to address disparities and health concerns of local communities. This position reports to the DCHD Deputy Director and supervises two Population Health Managers, the Wellness Line team, and additional Population Health Administration staff members. Essential Duties Provides leadership and strategic direction for the development of partnerships and collaborative programs to address priority determinants of health, as defined by the Community Health Improvement Plan or, if a plan has not been developed, by community coalitions, partnerships, or other community engagement. Provides leadership and strategic direction for the establishment and support of a countywide community health improvement coalition. Oversees the Health Department's role in the development of the community health assessment and the Community Health Improvement Plan. Identifies the need for additional topic or geographic coalitions and provides strategic direction for their development and support, including defining the health department's role. Represents the health department in community coalitions, partnerships, and committees. Provides strategic direction for, and operationalization of, community engagement in all public health programs. Collaborates with the community to identify and reduce systemic and structural barriers that perpetuate health inequities. Oversees chronic disease prevention programs. Collaborate through established partnerships to investigate or mitigate public health problems. Leads the Health Department's Emergency Preparedness efforts. Ensures capacity to respond to emergencies including a continuity of operations plan, surge capacity, training of Department responders, risk communications plan, communications with response partners and exercises and After Action Reports. Represents the public health emergency response effort with the County, State, and Federal agencies. Maintains the Department's public health emergency operations plan. Oversees the utilization and communication of pertinent data to the department and broader community and resource listings for individuals, equipment, and supplies needed for emergency operations. Maintains a department-wide approach for developing and implementing communication strategies designed to encourage actions to promote population health. Serves as a liaison to other health department divisions to understand data and policies needed to support program efforts. Monitors compliance with grant expenditures and assists in preparation of grant financial progress reports. Develops annual program budgets, in accordance with Department budget process. Performs other duties, tasks, and special projects, as required. Qualifications Bachelor's degree in public health or related field required, graduate degree in public health or related field preferred. Minimum of five years of public health experience including public health emergency preparedness experience and training. Two years of personnel management experience. The bachelor's degree and 5 years of experience may be substituted with an equivalent combination of education and a minimum of 10 years demonstrated public health emergency preparedness and/or community health improvement leadership experience. Valid driver's license and ability to pass a background check. Required to obtain, at their own expense, 3 forms of clearance, including PA State Police Criminal History Record Check; PA Child Abuse History; and FBI Criminal History Background Check including finger printing prior to employment. Delaware County requires all DCHD employees be up to date for their COVID-19 vaccination per CDC guidelines. Proof of vaccination must be submitted before a candidate's start date if hired.
