Job Description:

As a POC Nurse Coordinator, your primary responsibility will be to ensure adherence to quality standards and regulatory requirements. Your duties will include, but are not limited to, the following:

1. Quality Assurance Review:

Review and approve the daily Quality Assurance (QA) reports to ensure compliance with established standards and protocols.

Identify any discrepancies or issues and collaborate with relevant stakeholders for resolution.

2. Plan of Care (POC):

Ensure that PDN patients are recertified every 60 days in accordance with company policies and regulatory guidelines.

Conduct recertification and update Plan of Care (POC) for PCA patients every 60 days, adhering strictly to company protocols.

Ensure that PPEC patients are recertified every 180 days in line with company and regulatory requirements.

Coordinates the completion of Plan of Care in an accurate and timely manner, in accordance with policies and procedures, State and Federal requirements.

3. Start of Care (SOC):

Ensure that Start of Care (SOC) assessments are performed when the patient has obtained insurance authorization, in line with regulatory requirements.

4. Document Administration:

Complete the Plan of Care (POC) documentation accurately and promptly within the Axxess system or Acentra/EQ for authorization.

Obtain signatures from family members on the company's contract document during patient visits.

Effectively communicates with Director of Nursing Clinical Managers, Doctors etc., involved in the care planning process.

Conducts regular reviews of patient records.

Responsible for managing the administrative components of patient care plans.

Perform administrative duties such as completing medical forms, reports, etc., as necessary.

Any other duties that may be assigned.

Qualifications:

Must possess a current, unencumbered, active license to practice in the state of Florida.

Must have pediatric experience.

Strong understanding of healthcare regulations and compliance standards.

Proficiency in using healthcare software systems, particularly Axxess.

Proficiency in Microsoft suite

Excellent communication and interpersonal skills.

Detail-oriented with a commitment to accuracy and thoroughness.

Ability to work effectively both independently and collaboratively within a team environment.
