The Community Health Centers of Lane County (CHCLC) is seeking a Family Practice Physician to be part of an interdisciplinary team that provides comprehensive primary care within the Patient Centered Medical Home model at our brand-new South Lane Clinic. This mission driven individual will work as part of an interdisciplinary team at one of the CHCLC seven clinics. They will provide professional, comprehensive primary medical care and treatment services to a diverse population of patients in a community health center setting using a team-based approach. The Physician diagnoses and treats a wide variety of health conditions, provides education and referrals for patients as appropriate, and performs related duties as assigned.

About this Position


The Physician medically evaluates patients and assesses their medical status in the clinic; determines the clinical need for medications and prescribes medication as a part of a treatment program; discusses case management practices and techniques with health care professionals; provides information, consultation and assistance to other health care professionals as part of a patient centered medical home team and provides appropriate medical records documentation on all patients in accordance with policy.


The position may range from 0.8-1.0 FTE (32-40 hours/weekly) and may be eligible for additional bilingual compensation upon completion of a bilingual exam. All candidates are encouraged to apply and explore this opportunity.


Upon hire, successful candidate for Physician positions will be credited 40 hours to their leave bank.

In addition, employees in Physician positions who choose to open their panel capacity above the required minimum have the opportunity to earn $5,000 annually for each additional 50 patients, prorated by FTE, calculated and paid on a quarterly basis.

About the Location

The South Lane Clinic will offer medical, dental, and behavioral health care to the vibrant community of Cottage Grove, Oregon and surrounding areas. In partnership with Lane Community College, the clinic will provide training for local students interested in healthcare careers providing a collaborative approach to grow the healthcare workforce. The clinic is planned to open in the Summer of 2024 and will be established as a Rural Health Clinic (RHC).

About this Division

The Community Health Centers of Lane County (CHCLC) is a dynamic, growing organization that has been providing comprehensive primary care and behavioral health services to the citizens of Lane County since 2004. We operate six primary care clinics in which we provide a broad array of services to adults and children that are organized around care teams who work collaboratively to support our patients. Our integrated care teams include primary care providers, nurses, pharmacists, medical assistants, patient care coordinators, community health workers, and behavioral health specialists. This model of primary care prioritizes the patient, who has increased access to other services beyond general medical care at our clinics. Our care teams also help patients reach their optimal health status by addressing social determinants of health barriers to care such as transportation, food insecurity, or housing. The CHCLC provide affordable primary care for the whole family - infants, children, adolescents, adults, and seniors - as well as alternative services such as acupuncture, and preventative dental services. At the Community Health Centers of Lane County, we focus on integrated care that values the whole person. Our services encompass not only treatment and prevention of illness, but also health education and behavioral health services.

We also provide preventative dental services to children in schools, Head Start, and WIC locations throughout Lane County. CHCLC provides services to approximately 30,000 individuals each year, and is a primary source of care for uninsured and underinsured individuals. CHCLC is a Federally Qualified Health Center whose mission is to serve the community by offering excellent care and eliminating barriers to healthcare. Training:

  • Graduation from an accredited school of medicine with completion of a one-year internship and/or completion of residency program in a primary care setting by time of appointment.
  • An equivalent combination of experience and training that will demonstrate the required knowledge and abilities is qualifying.

  • State of Oregon Medical License including dispensing privileges- (copy of original) License must reflect Oregon address by time of hire
  • Diploma (copy of original is fine)
  • DEA Certificate (copy of original is fine, web document is not) DEA must reflect Oregon address by time of hire
