The PORT Health Services Outpatient Units primary purpose is to provide substance abuse, mental health and psychiatric services to individuals. Voluntary and involuntary persons are accepted. Services available to those in need include but are not limited to screening, assessment, intake, treatment, referral, psychiatric evaluation, group therapy, family intervention, community consultation, aftercare, and follow-up supports. Additionally, the facility provides a safe, supportive, therapeutic environment.Primary Purpose of Position:The primary purpose of the this position is to provide patients with services including screening, evaluation, diagnostic assessments, medication evaluations/prescription and management, assisting the treatment team with client treatment goals and person centered plans, treatment, after-care, and referral services. This position will provide stabilization and treatment services to mental health and substance abusing clients. Services provided to persons include but are not limited to giving medical diagnosis, ordering therapy and prescribing medications. This position will maintain accurate and up to date referral sources, and maintain a therapeutic environment and promote appropriate and healthy interpersonal behavior.Work Schedule:This position will maintain an average work schedule of 40 hours per week. Generally, working the hours between 8:00am-9:00pm, but flexibility will be expected depending on client and program need. In the event of inclement weather, the employee may be required to remain on the unit beyond their normal eight-hour workday.DESCRIPTION AND RESPONSIBILITY OF DUTIES:The Outpatient Unit provides screening, assessment, intake and intervention for individuals experiencing symptoms from substance abuse or psychiatric disorders and who are in need of outpatient treatment. The unit provides services to citizens from North Carolina's Eastern Region. Persons who present to this unit are adults/children of both genders and all races. The Unit provides information, screening, assessments, evaluations, referral, and intakes of new persons served. The position requires the ability to work in a mutually respectful partnership with persons served and their families to gain the skills and confidence to address any issues and problems they face. It also requires conviction about the capacity of people to grow and change ability to set limits and maintain the helping role and to intervene appropriately to meet the needs of the person served and/or family members.70% Direct Services The following are specific duties involving direct care or contact with the persons served, family members and the public:1. Obtain a thorough patient history, perform appropriate diagnostic and therapeutic procedures2. Medication evaluation, management and prescription3. Diagnostic assessments, evaluations and intakes4. Communicate with referral agencies and review pre-screening information for potential patients.5. Communicate with other treatment staff regarding observations and treatment interventions.6. Provide first aid, CPR or emergency intervention as needed.7. In the event of a disaster or emergency situation (e.g., hurricanes, flood, severe winter storms, widespread damage or human suffering) affecting any community within the catchment area, every employee shall perform after hours duties and special assignments as directed by the supervisor or other PORT Health Services management whether or not such duties or assignments are related to the employees regular duties.25 % Documentation: The following are specific duties involving documentation:1. Upon patients admission to the Outpatient Unit, the employee is responsible for obtaining and recording in a timely manner, pertinent medical and social information for the medical record which includes medical and progress notes, appropriate after care plans and termination/discharge plans in a timely manner.2. Will document any unusual and complex conditions or non-routine activities.5% Other Duties:1. Participation in training as a recipient and/or facilitator, staff meetings, committee work, team buildings, serve in assigned emergency shelter during natural disasters, and other duties as assigned.2. Serve as a supportive role model to the persons served and peers. Enforce program rules and regulations. Ensure that the interaction with each person served is therapeutic and educational in nature.3. Attend clinical staffing
