Experience REQUIREMENTS for both pt's and pta's
- 2 years Physical Therapy experience - Required
- BLS/CPR (AHA)
STATE LICENSE & Certification REQUIREMENTS Must be able to provide the following documentation prior to submission:
- Copy of License
- Proof of BLS/CPR (AHA)
- Provide 2 professional references
- Complete a Skill Checklist (Document will be provided during screening process)
- Current CV/Resume
The following is additional information regarding the requirements for onboarding should an offer be extended. We will work with you to gather this information, but please be aware it is required.
- Proof of MMR (2 doses or positive titer showing immunity)
- Proof of Varicella (2 doses, positive titer, or proof of history of chicken pox)
- Hep B (Proof of completed series or proof of immunity)
- TB Testing
- Influenza Vaccine
- COVID Vaccine
- Must be able to pass 10-panel drug screen and National Criminal Background search
- Physical/Health Examination
