Trusted Staffe is partnering with a thriving regional rehabilitation care facility in Wayne, PA, to find a dedicated Physical Therapist (PT) to join their team. This day shift (8:00 AM - 4:30 PM) position offers the opportunity to make a real difference in the lives of patients recovering from various conditions in a Skilled Nursing Facility (SNF) setting.

Responsibilities:

Conduct comprehensive patient evaluations to determine their needs and develop individualized treatment plans.

Provide evidence-based physical therapy interventions including therapeutic exercise, manual therapy, modalities, and gait training.

Educate patients and their families on their conditions, treatment goals, and home exercise programs.

Document progress in patient charts accurately and efficiently.

Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated care.

Participate in quality improvement initiatives.

Qualifications:

Doctor of Physical Therapy (DPT) degree from an accredited program.

Current licensure to practice physical therapy in the State of Pennsylvania.

Minimum of 1 year of experience in a skilled nursing facility setting preferred.

Strong clinical reasoning and decision-making skills.

Excellent communication, interpersonal, and collaboration skills.

Ability to work independently and as part of a team.

Compassionate and patient-centered approach to care.

Proficient in documentation and progress note writing.

Physical Requirements:

Ability to stand, walk, bend, and lift for extended periods.

Ability to transfer and assist patients with various mobility levels.
