JAG Healthcare Shelby is now hiring in the Therapy Department to add to our care team.

JAG Healthcare Shelby is a smaller, 45-bed facility offering both traditional skilled nursing and long term care as well as a cognitive deficit/behavioral resident care model. Physical Therapist Responsibilities:
Evaluating and treating patients
Creating and reviewing therapy treatment plans and administer prescribed PT treatment programs; help restore your patients function, relieve pain, and prevent disability following illness, disease, or injury
Conduct tests and measures to identify potential and/or existing problems your patients are facing
Complete re-examinations as needed
Develop discharge plans

Requirements:
Must be able to commit to at least 1-2 days every week for evaluations. We are flexible and are open to working evenings and/or weekends, but not mandatory.
Physical therapy license to practice in the state of Ohio.
Must have knowledge and/or experience working with geriatric population and some basic psych diagnosis such as dementia, parkinson's, schizophrenia, bipolar disorder etc.
Must be a team player, but also can work with minimal supervision.
Ability to make judgements regarding patient care and clinic operations
