Creekside Post-Acute has been serving San Jose, California for over 50 years. It is a 130-bed that specializes in short-term rehabilitation and skilled nursing. We are seeking a dedicated and experienced physical therapist to join our team in a skilled nursing facility.

RESPONSIBILITIES:

Assess and evaluate patients' physical condition and develop personalized treatment plans
Administer various physical therapy treatments, such as therapeutic exercises, gait training, and manual therapy
Educate patients and their families on effective exercises and techniques for home-based care
Monitor patients' progress and adjust treatment plans as necessary
Collaborate with healthcare professionals, including occupational therapists and doctors, to ensure comprehensive care
Document patients' progress and maintain accurate and organized records
Stay up-to-date with the latest developments in physical therapy and participate in continuing education activities

REQUIREMENTS:

Bachelor's or Master's degree in Physical Therapy from an accredited program
State licensure or eligibility for licensure as a physical therapist
Is in good standing with all regulatory agencies and licensing boards is required
Interest in working with the geriatric population
Strong knowledge of anatomy, physiology, and medical principles related to physical therapy
Excellent communication and interpersonal skills
Ability to work effectively both independently and within a team
Strong problem-solving and critical-thinking abilities
Proficiency with electronic medical record (EMR) systems is a plus

EXPERIENCE:

New passer/graduates are welcome
With Skilled Nursing or Long-Term Care experience is preferred
Experienced Physical Therapists (PT)
