Duties:
Provides Physical Therapy services to patients in a safe and effective manner.
Evaluate patients within 72 hours of physician referral
Provides treatment in accordance with established standards of practice.
Coordinates treatment plan with patient, family, and other team members.
Implements and conducts treatment as outlined in the treatment plan
Monitors patient's response to intervention and modifies treatment
Supervise Physical Therapist Assistants
Participate in Patient Care Conferences as needed
Participate in inservice training program for other staff in the facility
Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy
Record treatment charges daily per facility policy and procedures
Instruct patient's families or nursing staff in maintenance programs
Participate in discharge planning
Must-Have Qualifications:
Bachelor, Masters, or Doctorate of Physical Therapy from an accredited program.
Active license to practice as a Physical Therapist in Arizona.
Current level one Arizona Fingerprint card
Current CPR
This position is open to PTs at all experience levels, one year experience in skilled nursing preferred
