Position Overview:

The Physical Therapist (PT) will play a critical role in assessing, planning, and implementing physical therapy treatments to improve patient mobility, alleviate pain, and prevent or limit permanent physical disabilities in our residents. This position requires a compassionate individual who is committed to patient-centered care and capable of working independently as well as part of a multidisciplinary team.

Key Responsibilities:
  • Conduct comprehensive assessments to establish individualized care plans for residents.
  • Implement physical therapy treatment plans and monitor progress.
  • Collaborate with the healthcare team to ensure holistic care.
  • Educate residents, families, and caregivers on exercises and treatment plans.
  • Document patient care services by charting in resident and department records.
  • Maintain patient confidence and protect operations by keeping information confidential.
  • Ensure operation of equipment by completing preventive maintenance, following manufacturer's instructions, troubleshooting malfunctions, and requesting repairs.
  • Update job knowledge by participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations.
Qualifications:
  • Bachelor's or Master's degree in Physical Therapy from an accredited program.
  • Current Alabama state license as a Physical Therapist.
  • Experience in a skilled nursing or rehabilitation facility preferred but not required.
  • Excellent communication and interpersonal skills.
  • Strong analytical and problem-solving skills.
  • Ability to work independently and as part of a team.
