Salary: Competitive, based on experience Job Summary: Seeking a dedicated Physical Therapist to join our Client's program. This role involves evaluating participants' mobility and physical conditions, developing and overseeing individual and group exercise programs, and providing comprehensive physical therapy. Essential Duties and Responsibilities:
  • Assessment: Collect and document patient data, identify needs, and prioritize care.
  • Planning Care: Develop care plans in collaboration with other disciplines, set measurable goals, and make necessary referrals.
  • Implementing/Managing Care: Provide therapy according to standards, educate participants and families, and document all treatments accurately.
  • Participant/Family Education: Coordinate care delivery, train family members and staff, and conduct home visits as needed.
  • Evaluation: Record and evaluate care delivery, make observations, and implement rehabilitation measures.
  • Leadership: Supervise program aides, integrate cost-effective measures, and participate in quality improvement processes.
  • Interdisciplinary Team: Actively participate in team meetings and care planning.
  • Other: Perform additional duties as assigned, within the scope of practice.
Requirements:
  • Bachelor s degree in Physical Therapy (pre-2000) or Master s degree (post-2000).
  • Licensed Physical Therapist in the state of Indiana.
  • Experience with the geriatric population preferred.
  • Reliable transportation, valid driver's license, and required auto insurance.
  • Must pass a comprehensive background check and meet health clearance requirements.
  • Effective communication and teamwork skills.
Work Environment and Physical Demands:
  • Work in a busy rehabilitation gym and Adult Day Health Center.
  • Significant hands-on care, including lifting, carrying, pushing, pulling, stooping, kneeling, and crouching.
  • Ability to stand, bend, walk, and sit for extended periods, and work at a computer.
  • Occasionally assist with lifting participants and helping them turn, walk, or stand.
