Physical Therapist

Responsibilities:
Develop and implement physical therapy plans of care ensuring quality and safe delivery of physical therapy home health care services.
Work directly with patients to achieve maximum physical recovery.
Evaluate effects of therapy treatment and communicate patients' progress.
Record and document patient care services.
Promote education for clients, client families and the community.
Coordinate provision of physical therapy home health care services that reflect VNCC's philosophy and standards of home health for assigned clients

Qualifications:
Current PT licensure in MA required.
Two (2) years experience in home health care, elder services, and/or public health preferred.
Strong assessment skills needed.
Excellent communication skills with a customer service orientation necessary.
Ability to build rapport with patients.
Ability to handle physical workload.
Work with a friendly and supportive team of seasoned clinicians at an agency that understands the need for work/life balance.
