Physical Therapist

Position: Physical Therapist
Locations: Albany, Indiana
Contract Duration: 14 weeks contract with a possibility of extension.
Openings:1
Pay Rate: up to $50/hr (Travel candidates only outside 50 mile radius)
Work Schedule: Monday to Friday
Shifts: 8 hours Day Shift
Facility would like to offer a $3,000 completion bonus to the traveler after 16-weeks OR $5,000 Completion bonus after a 26-week contract. -Terms: 576 hours in a 16 week period with no more than 2 call offs or tardies ; 936 hours with no more than 3 call offs or tardies.

Patient Population: vertigo, post op, typical ortho
Productivity expectation is 60 visits/ week; Will see 12-14 patients/day; Double-booking of follow up visits expected.
