We provide Part B Outpatient Therapy Services in the ALF/IL/Home Setting

Position Summary : This position is responsible for providing outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or in a patient's room in a SNF/Dementia-care facility, in an assisted living facility, or in an independent living facility. This position will evaluate and treat in the most functional environment which is the patient's home and assesses the need for, develop, and deliver physical therapy interventions to facilitate rehabilitation both on an intermittent and maintenance-based scale. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. The Physical Therapist will implement individualized therapy program designed to restore, reinforce, and enhance performance and optimize independence. In addition, this position instructs, educates and trains patients and caregivers in those skills and functions essential in promoting quality, independence, and productivity, in order to diminish or correct movement disorder pathology.

Essential Functions :
  • Works closely with the Regional Director of Therapy Operations and Program Manager to ensure proper execution and maintenance of the Wellness Programs
  • Screens, examines and evaluates patients, including history and systems review
  • Provides thorough HEP education to patients and caregivers in a manner they can understand, and plans appropriately for discharge when medical necessity no longer indicates skilled therapeutic care.
  • Makes clinically necessary referrals to other disciplines within care partners as well as other agencies in order to address the most complex and comprehensive needs of the patient.
  • Consults with other members of the Health & Wellness team to select the most appropriate therapeutic regimen consistent with the objective findings, functional needs and capabilities of each patient.
  • Adapts program and/or plan of care according to the needs of individual patient.
  • Orders supplies and equipment (DME) as necessary
  • Maintains, identifies, and reports the need for repair of equipment as necessary
  • High quality patient care. Ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
  • Participates in departmental initiatives and demonstrates effective communication with all team members.
  • Facilitates professional growth, development and overall improvement of departmental performance.
  • Performs other professional duties associated with the care of the patient, development of discipline-specific therapy services, and design of program delivery.
  • Application of appropriate tests, measures (FOMs) and therapeutic intervention.
  • Identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care.
  • Follows proper continuing education requirements and assures up to date licensure.
  • Services include but are not limited to application of therapeutic techniques in areas such as:
o Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuro-motor development training, relaxation, strength, power, and endurance training

o Functional training

o Manual therapy

o Airway clearance techniques

o Integumentary repair and protection

o Use of electrotherapeutic, physical agent and mechanical modalities

o Education on Chronic Disease Management

o Falls Assessment and Interventions to reduce fall risk

o Case Management in the home and community

Qualifications:
Minimum Qualifications
  • Valid State Physical Therapy License/ Registration REQUIRED to start
  • Cardio-Pulmonary Resuscitation (CPR) certification required
Preferred Qualifications
  • Dementia experience
  • Geriatric experience
  • Interest and/or experience in lymphedema
  • At least 1 year of geriatric experience
