Job Details

Responsibilities:

• Evaluate and assess patients' physical conditions, medical histories, and treatment goals to develop individualized treatment plans.
• Implement and administer various physical therapy techniques and exercises to promote mobility, strength, and overall well-being.
• Monitor and track patients' progress, adjusting treatment plans as necessary to ensure optimal outcomes.
• Educate patients and their families on proper techniques for performing exercises and managing their conditions at home.
• Collaborate with interdisciplinary healthcare teams to coordinate and integrate care plans, ensuring a holistic approach to patient rehabilitation.
• Document and maintain accurate patient records, including initial assessments, progress notes, and discharge summaries.
