208 Mill Road Physician Day shift ASSOCIATE MEDICAL DIRECTOR OF COMMUNITY INTEGRATION, SOUTHCOAST HEALTH NETWORK Job Description The role of the Associate Medical Director of Community Integration, Southcoast Health Network (SHN), is critical to the success of the organization in developing and supporting the implementation of a coordinated community integration strategy that addresses social determinants of health (SDOH). The Associate Medical Director will be responsible for organizing such strategy development and implementation in collaboration across departments within Southcoast Health, including Primary Care, Community Benefits, and Government Affairs. The Associate Medical Director will represent SHN as a community integration leader within and outside the organization, and meaningfully engage with stakeholders to ensure that clinical operations support successful closure of SDOH gaps via productive partnerships with community organizations. It is expected that this role will be 85% clinical. Direct reports: N/A Reports to: Chief Transformation and Innovation Officer Key relationships: CFO, SVP Physician-in-Chief Primary Care, Executive Director of Business Intelligence and Operations, Executive Director of Integrated Care Management, Manager of Operations, Manager of Care Navigation, Director of Organizational Culture and Engagement, Primary Care leadership, Post-Acute Care Network leadership, SHN Analytics, SHN Ambulatory Quality, Community Benefits leadership, Government Affairs leadership, external relationships including, but not limited to, clinical and non-clinical community providers and payers Desired experience: Relevant experience with community-based / social services organizations, health care leadership experience, MPH/MBA or related degree preferred. SCOPE AND PURPOSE OF THE JOB: COMMUNITY INTEGRATION STRATEGY DEVELOPMENT AND IMPLEMENTATION In partnership with SHN and Community Benefits, ensure that key community partners (e.g., social service organizations, federally qualified community health centers) are effectively integrated in to Southcoast's clinical operations and health equity strategy. Develop trusting relationships with key community partners and identify areas for improvement with existing relationships, identify new strategic relationships Identify opportunities for integration between Southcoast and community partners, including, but not limited to, multidirectional communication, referrals, and effective SDOH (social determinants of health) gap closure Develop a measurement system for tracking impact of community integration strategies on clinical outcomes and use this data to drive program development. Serve as advisor and/or inform Community Benefits reporting Participate in ongoing health and social needs assessments and community health improvement planning Identify funding opportunities to support community integration strategy and contribute to grant applications / philanthropic proposals in pursuit of funding. Where appropriate, act as liaison between SHN, Community Benefits, and community partners to ensure strategy alignment and optimal implementation IMPLEMENTATION AND PROCESS IMPROVEMENT Collaborate across Southcoast entities and affiliated practices, including primary care, specialty, hospital, VNA, and post-acute care to implement activities tied to community integration strategic priorities. Engage in process development, improvement, and problem solving to maximize population health performance Ensure optimal deployment, target populations, and workflows for community-facing resources, e.g., Community Health Workers Host and facilitate network-wide, community-wide meetings and seminars on the topic of health equity and community integration Continuously monitor performance and escalate trends to appropriate leadership team(s); identify and implement interventions to improve health equity outcomes, quality, and patient experience LEADERSHIP Effectively communicate with and educate Southcoast clinical and non-clinical colleagues, from across the continuum of care, on community-related issues and initiatives; review performance regularly with Southcoast and affiliated providers. Serve as a physician advisor to SHN's clinical and non-clinical programs (e.g., quality, care management, post-acute care, social services) Serve as SHN medical staff representative to various committees and workgroups and/or lead such committees and workgroups, as needed The Associate Medical Director will be expected to provide six (6) hours (0.15 FTE) per week performing the duties listed above. In consideration of providing these services, Corporation shall pay Physician One Hundred and Forty-Five Dollars ($145.00) per hour up to a maximum of $46,800 per Compensation Period, subject to the timely submission of timesheets.
