Job Description SCOPE OF POSITION
The Clinical Pharmacist shall be responsible for the provision of clinical pharmacy services/pharmaceutical care encompassing the service's Scope of Care for the patients of the entity and shall practice under the direction of pharmacy leadership and the guidance of professional pharmacy standards. He/She will provide information about medication therapy in both formal and informal settings to healthcare providers, including physicians, as well as patients and families. The Clinical Pharmacist will participate as appropriate in the education and training of staff pharmacists, resident physicians, patient care personnel and pharmacy students selecting the entity as clinical clerkship sites. The Clinical Pharmacist serves in a supervisory role when assigned and/or in the absence of supervisory personnel. Personal characteristics necessary for successful performance in this position include flexibility, attention to detail, initiative, cooperation, as well as a commitment to continuing education to upgrade skills in a constantly developing profession. This individual should demonstrate good communication and interactive skills, oral and written, a positive outcome/results orientation (including accountability for results) and the ability to function successfully as part of intradepartmental or multidisciplinary teams. He/She should be able to provide pharmaceutical care for neonate, infant, pediatric, adolescent, adult and geriatric patient age groups.

He/She should support the Ballad Health Mission, Vision, and Values and shall be committed to the improvement of care quality via the organization's Performance Improvement Initiatives.

Clinical Pharmacists assigned to Teams/Programs will collaborate professionally and effectively with team members (including physicians) regarding patient care needs and with a goal of positive, patient-oriented outcomes. This collaboration will include regular rounds with the Team for development of patient management plans.

Access to patient information may be needed to fulfill the responsibilities of this role.

EDUCATION AND EXPERIENCE
This individual must be a graduate of a college of pharmacy accredited by the American Council for Pharmaceutical Education. While a PharmD degree is desirable it is not required but the successful candidate must have documented recent experience and proficiency in the delineated skills. Previous completion of an appropriate or related residency or fellowship is preferred. Consideration will be given to pharmacists with the following experience in lieu of completion of residency or fellowship:

Attainment of Board Certification status in Pharmacotherapy plus minimum of 2 years fulltime clinical experience in a similar setting

OR

5 or more years of fulltime clinical/clinical focused experience in a similar setting.

Experience should include, but not be limited to, clinical consultations/review and medication usage evaluation for patients of all age groups. Demonstrated proficiency and experience in the areas of nutrition support and pharmacokinetic dosing is required. Capabilities/knowledge must also be exhibited in the use of related information systems hardware/software. Previous experience/proficiency in the provision of education and training for pharmacy students is desirable.

Individuals applying for/participating in the Team/Program must manifest proficiency in the treatment of critical care patients based on previous experience and/or specialized training. (Input from pertinent healthcare providers, including physicians, may be solicited.)

Demonstration/documentation of positive interactive skills with other healthcare providers (including, but not limited to, physicians, physicians' assistants, nurse practitioners, nurses, and pharmacists) is required, as well as a commitment to maintaining those skills. Negative record or assessment in this area eliminates candidates who meet educational and experiential requirements.

The pharmacist must hold or be eligible for licensure by the appropriate State Board of Pharmacy (Licensure must be obtained within 90 days of hire date).
BLS required, ACLS and/or PALS may be required based upon assigned job duties
