Position Summary: Provides accurate, safe, efficient and cost effective pharmaceutical care, either directly or indirectly through the supervised efforts of support staff to the patients of GCMC.

Education, License & Cert:

  • PharmD or Bachelor of Science in Pharmacy with an active New York State Pharmacy License.
  • Active New York State Pharmacy License.

Experience: Completion of an accredited clinical residency and demonstrated ability to achieve positive patient outcomes in a similar environment preferred. Active membership in local and national pharmacy organization is preferred.

Essential Functions

1) Team Member Expectations

a. Understands and adheres to GCMC's compliance standards as they appear in GCMC's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.

b. Keeps abreast of all pertinent federal, state and GCMC regulations, laws, and policies as they presently exist and as they change or are modified.

c. Demonstrates skill in area of responsibility. Skill is defined as excellence in quality and quantity of work produced.

d. Develops a spirit of teamwork. Recognizes when help is needed, provides assistance without being asked and does not judge when help is asked for by others.

e. Learns from mistakes and helps to foster an environment of constructive feedback.

f. Works to ensure patient safety is maintained in all aspects of services provided by the department. Raises concerns without fear of negative feedback. Contributes suggestions to improve unsafe situations. Executes the "stop the line" process when needed to prevent patient harm.

g. Participates effectively in department meetings. Arrives on time, respects the opinions expressed, and works to identify a solution that optimizes patient care or improved performance of the department.

h. Works to improve the practice of pharmacy by working with and training new employees, pharmacy interns and students.

i. Imparts clinical pharmacy knowledge by presenting pharmaceutical or medication use safety in-services for clinical staff or community educational programs. Standard is 2 per year.

2) Operational and Service Excellence Expectations

a. Maintains all required records including controlled substance records and all appropriate department forms.

b. Processes physician orders quickly and accurately including appropriate instructions and timing to nursing to ensure safe and efficient medication administration.

c. Appropriately questions, rectifies and documents response to any medication related problem encountered during order processing, ie duplications, allergies, non-compliance with policy, etc.

d. Follows policy and procedure order processing for high risk patients (pediatrics) and high risk medications-chemotherapy, anticoagulants, etc.

e. Ensures medications are being distributed in a timely fashion meeting department expectations and patient needs. Checks medications timely to prevent distribution delays.

f. Strives for zero defect production level in medications dispensed from pharmacy

g. Ensures all tasks are complete at the end of the shift or communicates items left to next shift

h. Prevents Medication Occurrences: performs the appropriate documentation of Adverse Drug Reactions and medication occurrences to ensure system problems are identified and resolved. Standard is 9 reports/quarter.

i. When in the IV room, works to ensure products are mixed accurately and timely according to developed processes and expectations.

j. When in the IV room, works to ensure 797 requirements are being adhered to.

k. Follows policy and procedure for clarifying orders- all clarifications should be resolved within 24 hours.

3) Clinical Excellence

a. Demonstrates understanding of the patient and his/her condition or disease and its treatment.

b. Appropriately handles dosing, monitoring, and pharmaceutical care of high risk patients: chemotherapy and pediatrics.

c. Performs the appropriate documentation of interventions. Goal is 200 interventions/quarter.

d. Antibiotic Stewardship: Ensures all antibiotic usage is appropriate. Streamlines and de-escalates therapy to reduce the development of resistance and hospital acquired conditions. Documents in interventions.

e. Dose Adjustments: Ensures appropriateness of medication regimens, taking into account medication indication, patient age, weight and renal and hepatic function. Documents in interventions.

f. Reacts appropriately to serum level, INR and all other pharmacy-generated reports and alerts.

g. Ensures appropriate DVT prophylaxis. Documents interventions.

h. Participates actively and effectively in multi-disciplinary rounds.

i. Demonstrates active participation in the Medication Reconciliation Process by obtaining med histories when needed, resolving discrepancies and documenting involvement.

j. Responds to drug information questions using appropriate references. "Appropriate references" are defined as those approved by the department.

4) Financial Success

a. Actively minimizes waste within the pharmacy through appropriate practices.

b. Performs cost savings interventions including changing ordered therapy based on pharmacy inventory.

c. Ensures appropriate patient charging- is not identified as a cause of charge audits.

d. Identifies cost-saving or operational efficiency ideas that will contribute to the well-being of the patient, department or hospital. Goal: 1 idea/pharmacist/year.

5) Strategic Growth

a. Demonstrates his/her contributions to promoting the success of the department yearly goals through the completion of individual goals.

b. Contributes to the development of the Pharmacy Department, either by special projects or in-services.

c. Demonstrates continued learning, is current with new advances in pharmacy.

d. Displays genuine interest in the profession of pharmacy through participation in local, state or national professional organizations.

e. Performs other duties as requested and observed by manager or supervisor.

The pay for this position ranges from $47.14-$74.67 per hour.
