SALARY RANGE: $450,000 - 650,000 annually Requirements:
- Possess/Attain active, unrestricted AZ license
- Board Certified
- Possess/Attain AZ DEA
- Clean DEA/Malpractice/NPDB (no unresolved suits/cases, no large payouts in previous 7 years)
