Job DescriptionSummary

The Pharmacist isresponsible for clinical review of medication orders for thehospital and serves as a clinical resource to Providers, Nursingstaff and other departments. Oversees the preparation anddispensing of medications to inpatients, emergency room andambulatory clinics. Participates in multi-disciplinary clinicalrounds on patient units and provides clinical information tomedical staff. Maintains standards set by the department andadheres to policies and procedures of the Pharmacy to meet thestandards of Joint Commission. Oversees the Pharmacy Sterile andNon-Sterile Compounding processes. Assists in Quality and Safetyimprovement initiatives under direction of the PharmacyAdministration Team.

JobDescription

ESSENTIALFUNCTIONS

1. Performs clinical review ofmedication orders from prescribers throughout the hospitalincluding Emergency room, Critical Care unit, Pediatrics andMedical Surgical units. Intervenes and providesrecommendations to providers for drug dosing errors, druginteractions, allergies or drug contraindications. Suggestsappropriate or alternate drug therapy to the providers whenneeded.

2. Responsible foroversight of preparation and distribution of medication ordersfilled for the in-patient, outpatient and ancillary areas. Responsible for ensuring medications contain the correctlabeling and drug components. Responsible for oversight ofSterile, Non-Sterile and Hazardous compounds and ensures adherenceto Pharmacy policies.

3. Responsiblefor reviewing patient medication profiles and monitoring clinicalparameters to ensure the proper choice of drug, dosage form, dose,length of therapy and interaction potential of the prescribed drug. Reviews and completes Pharmacy algorithms for antibioticdosing and Pharmacy drug interchanges for medications not onFormulary.

4. Responsible fortraining, supervising, educating and checking the work of PharmacyTechnicians and students. Serves as preceptor to pharmacy studentswhen needed.

5. Participates inMultidisciplinary rounds on Nursing Units when needed. Maintains clinical relationship with providers and providesclinical information when needed. Reviews patient clinicalrecords and makes appropriate recommendations when needed.

6. Responsible for maintaining aworking knowledge of, and conforming to legal requirementsconcerning the distribution of drugs, as well as good workingknowledge of recent developments in the field of pharmacy.Completes required continuing education as required by law.Completes yearly competencies assigned by the hospital.

7. Assumes "charge pharmacist" responsibilities asrequired, for the coordination of weekend and evening shifts whichinclude supervising the part-time pharmacists and supportivepersonnel, assigning work and delegating tasks andresponsibilities, and for handling problems that arise during theabsence or unavailability of the Director of PharmacyServices.

8. Responsible tofunction as pharmacy liaison to assigned nursing units for thepurpose of handling routine, day-to-day issues. Performsmonthly floor inspections as assigned.

9.Responsible for the distribution, control and record keeping of allcontrolled substances. Participates in weekly inventories ofcontrolled substances when needed.

10.Responsible for conducting, when asked to, in-service educationlectures for medical staff, nurses, pharmacists, students and otherhealth care personnel regarding drugs, procedures andpolicies.

11. Responsible forintradepartmental communication by staff meeting and in-serviceattendance. Also responsible for knowing content of departmentalstaff meeting minutes andmemos.

12. Safety Awareness -Fosters a Culture of Safety through personal ownership andcommitment to a safe environment. Participates in safetyforums and meetings when asked upon. Participates in qualityimprovement and safety initiatives when asked.

13. When asked to, is responsible for covering the GERM pagerin accordance with Department practices set forth by the AMSservice.

JOBREQUIREMENTS

Minimum Education-Preferred

Bachelor'sof Science Degree in Pharmacy from an accredited institutionpreferred.
Advanced degree preferred or enrollmentin advanced degree program.

Minimum WorkExperience

Minimum of one to threeyears experience as a Registered Pharmacist in a hospitalpreferred.

A.S.H.P.accredited residency preferred.
