Category Social Services Description JOB SUMMARY: The Peer Specialist will work with people experiencing homelessness that are survivors of domestic violence, dating violence, sexual assault, human trafficking, or stalking. The PS will work with the Housing DV-Plus team (Case Managers and Housing Specialist) to identify the ongoing supportive needs of the clients. This will include securing permanent housing with the intent of maximizing self-sufficiency and moving the individual and/or families into housing as quickly as possible. The team will allocate its resources to address the needs of the client based on the assessment such as housing placement, substance use disorder (SUD), concerns including mental health and holistic services. DUTIES AND RESPONSIBILITIES:? Collect project-related data through an initial orientation with clients. Obtain necessary releases and consents and advise the client about their rights and responsibilities. Assist the CM with maintaining case files on each participant. Maintain progress notes on each client interaction and participate in staffing meetings to aid in client remaining stably housed and their progress. Meet regularly with individuals/families assisting them in identifying resources, developing action plans, and securing services. Assist the CM with maintaining the care management plan to integrate physical and behavioral healthcare needs and establish goals. Work closely to provide and support the individual in building life skills and attaining short and long-term goals. Provide ongoing support to address barriers that impede client's ability to remain stably housed. Provide information and appropriate referrals and facilitate access to other resources as required. Work individually with each client on-site, assisting with transportation to supportive services. Facilitate communication and coordination between client and referred organization. Position Requirements SKILLS AND ABILITIES: Must have lived experience and advocate for DV survivors and families for at least three years within the community. Must have work related experience to either of the following: domestic violence dating violence sexual assault human trafficking stalking Must have a teamwork approach and work cohesively with other departments. Support current organizational initiatives and exhibits creativity and flexibility in times of change. Seeks formal and independent learning activities to maintain and develop professional skills and knowledge. Able to work flexible hours and attend appointments with participants if needed. Able to follow program guidelines of confidentiality as required. Able to easily build rapport with clients. Must be a strong patient advocate maintaining a relationship that fosters trust and understanding. Serve a consistent, positive, and appropriate role model to the client(s) served. SUPERVISORY RESPONSIBILITIES: None. QUALIFICATIONS: Basic computer skills (such as Microsoft Suite); Knowledge of HMIS (Homeless Management Information System) and agencies. Worked experience with emergency shelter, transitional housing programs, and/or other residential programs. Knowledge and experience with the following: domestic violence, crisis intervention, child abuse, substance abuse, mental health, welfare rights, case management, support group facilitation, sexual assault, employment services, child development, and/or positive parenting skills.
