Overview Hours: Monday-Friday 8:00am-5:00pm Enhabit Home Health & Hospice is hiring a Patient Services Coordinator (Scheduler) to join our team. The Patient Services Coordinator will schedule patients for field clinicians. They will communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care. This position will also manage the on-call notebook and hospitalization logs to enhance communication among stakeholders. LPN license required Responsibilities Scheduling home visits for the field staff each day and assisting with the coordination of patient care. Manage and complete patient schedules; assign visits timely to ensure completion of patient care, compliance with orders, and allowance for staff to appropriately prioritize visits daily. Ensuring geographic scheduling to reduce drive time and to enhance staff satisfaction. Accumulating admissions and intake information. Communicating with physicians about orders and patient progress information. Qualifications Must possess a high school diploma or equivalent. Must be an LPN/LVN licensed in the state in which he/she practices, with at least one year of clinical experience in a healthcare setting Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Previous experience in home health, hospice, or pediatrics is preferred. Requirements Must possess a valid state driver license Must maintain automobile liability insurance as required by law Must maintain dependable transportation in good working condition Must be able to safely drive an automobile in all types of weather conditions For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license Additional Information Enhabit Home Health & Hospice is an equal opportunity employer.
