The Clinician provides home and community-based behavioral health services such as, assessment, triage, and treatment to individuals diagnosed with mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions. Services will be provded to at rsik youth and their families. In addition the Clinician will also complete intakes and provide case management services where needed. Clinicians may be subject to work extended hours and weekends to address operational needs.

Mileage Reimbursement


  • Conducts intakes and admissions to facilitate access to services.
  • Performs diagnostic evaluations and assessments of strengths, needs, functional impairments, symptoms at regular intervals to ensure services are individualized and appropriate to each individual's level of care.
  • In partnership and using the trauma informed care and recovery-based perspective Clinician creates and regularly reviews treatment plans to ensure services continue to be appropriate and optimal.
  • Provides therapeutic treatment services and/or interventions such as, but not limited to; skills training, psychoeducation, psychotherapy (individual, family, and/or group), crisis services, case-management and linkages and referrals.
  • Document in a timely, accurate and complete manner that meets regulatory policies including, but not limited to, Southwest Key Programs.
  • Adhere to all confidentiality laws, policies and regulations.
  • Responds to corrections or actions needed to submit accurate funding invoices.
  • Supports the establishment of cohesive multi-systemic alliances among community stakeholders.
  • Attending scheduled supervision meetings with immediate supervisor.
  • With time request attend training, workshops, and meetings.
  • Be in good standing with the respective licensing board.
  • Other duties as assigned or required.

  • Maintain open, consistent, and effective communication with program staff , supervisor and community stakeholders
  • Adhere to all required trainings and maintain required certifications and licensures
  • Able to react to change productively and handle other essential tasks as assigned.
  • Able to work weekends and evenings as required and/or needed

  • Bilingual in English/Spanish.
  • Graduation from an accredited college or university with a master's degree with specialization in Human/Behavioral/Social Services or related fields is required.
  • Licensed in respective field (to include, but no limited to: LPC -Associate (License Professional Counselor), LMSW (Licensed Master Social Worker), LPC, LCSW or LMFT
  • Must possess a valid Texas driver's license, registration and maintain state mandated car insurance.
  • Computer literate with working knowledge of Microsoft Windows (Microsoft Word, Excel), variety of online database systems.
  • Possess good verbal and writing skills.
  • Clear background check from appropriate entity.

Must be able to obtain First Aid Certification (CPR). Must be able to verify client's whereabouts by driving, sitting, standing and walking for extended periods of time. Daily use of various office equipment required. Bending, stooping, climbing up stairs and lifting up to 15 lbs. required to complete daily tasks.


This is a program which requires daily contact with clients. Work shifts are subject to change. Noise levels vary from mild to moderate. This position may require extended periods of time in an automobile. Exposed to prevalent weather conditions.
